Please call us at: <a href=\"tel:+13147290077\" class=\"phone \"> <span class=\"phNum\">(314) 729-0077<\/span><\/a><\/strong><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Through your Patient Portal you will be able to:<\/h2>\n\n\n\n<ul>\n<li>View your upcoming appointments<\/li>\n\n\n\n<li>View your visit notes and treatment plans<\/li>\n\n\n\n<li>View your medical records and medications<\/li>\n\n\n\n<li>View your sleep study interpretations<\/li>\n\n\n\n<li>View your CT and treatment planning interpretations (images not included)<\/li>\n\n\n\n<li>View your pathology results<\/li>\n\n\n\n<li>Update your medical history<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">How to activate your patient portal:<\/h3>\n\n\n\n<p>You will receive an email to activate your Patient Portal. Please activate your patient portal within 72 hours. If you need help, please call: <a href=\"tel:+13147290077\" class=\"phone \"> <span class=\"phNum\">(314) 729-0077<\/span><\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">To help you get started, here are simple step-by-step instructions for common tasks:<\/h2>\n\n\n\n<p>Download PDF Instructions<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">How to access your visit notes:<\/h3>\n\n\n\n<ul>\n<li>Select &#8216;Appointments&#8217; from the top navigation bar<\/li>\n\n\n\n<li>Select &#8216;Past Appointments&#8217; from the side bar<\/li>\n\n\n\n<li>To view your Visit Notes, select the blue hyperlink with the date and time of your visit<\/li>\n\n\n\n<li>To download or print the notes, select the &#8216;Download&#8217; or &#8216;Print&#8217; buttons in the document viewer<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">How to access tests and results:<\/h3>\n\n\n\n<ul>\n<li>Select &#8216;Results&#8217; from the left bar of the appointments screen OR select &#8216;Tests and Results&#8217; from the top navigation bar.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">How to request records that are not accessible through the Patient Portal:<\/h3>\n\n\n\n<ul>\n<li>Select &#8216;Messages&#8217; from the top navigation bar<\/li>\n\n\n\n<li>Select &#8216;Compose Message&#8217;<\/li>\n\n\n\n<li>In the &#8216;To&#8217; field, type in or select &#8216;Medical Records&#8217; from the drop-down menu<\/li>\n\n\n\n<li>In the message field, indicate which records you would like our staff to process for you<\/li>\n\n\n\n<li>Select &#8216;Send&#8217; to send your request to us<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Accessing records outside of the Patient Portal:<\/h3>\n\n\n\n<p>If you are having difficulty accessing your records in the Patient Portal, you can also request records by filling out this form:<br>Request Medical Records<\/p>\n\n\n\n<p>Did you know? Once you activate your Patient Portal, you can also access your medical records on your smartphone.
